3Prepare your USPS Form 1583

Federal rules require this form before we can accept mail for you. You sign it in front of the person who checks your ID, not online. You can answer it here, so the counter visit is a signature rather than a form. Please do not send ID numbers or photos. Just tell us which documents you will bring.

Opening it for a business?

A business receiving mail here needs its own Form 1583, naming where it is registered.

This part cannot be done by web form. USPS Form 1583 requires a person to check your two IDs, either our staff in the shop or a notary who verifies your identity remotely. This is a federal rule for every mail-receiving agency, and we will walk you through it.
